RTO (Regional Transport Office) is a government organization which manages the database of drivers and vehicles of all the states in India. The four primary functions of an RTO include:
Issuing the driving license.
Coordinating and managing a collection of vehicle excise duty.
Selling personalized registrations.
Examining the insurance certificate that is provided by the owner and also conducts the emission test.
The government has launched an RTO forms list which can help the applicant in identifying the information and inputs that he/she needs to supply to the RTO. The list of RTO forms are divided into two sections; one for vehicle registration and the other section is meant for driving license. The details linked with the distinct types of forms are mentioned below:

Form 1 |
In the category of driving license, the first form is Form 1. Form 1 is also known as ‘Application-Cum-Declaration as to Physical Fitness’. This application is asked to be filled by the RTO to know whether the applicant has any form of disability. |
Form 1A |
The next form in the category is the Form 1A or ‘Medical Certificate’. This form will include the results of a thorough examination conducted by a medical practitioner who is appointed by the state government. The information filled by the candidate in Form 1 is verified using the examination. |
Form 2 |
In Form 2 or ‘Application for the grant of learner’s license’, the applicant has to fill the type of motor vehicle he/she is applying for. Other information to be filled include the driving licence history of the candidate such as ownership of any previous licenses, disqualifications, etc. There is a separate section in the form for minor ownership of a driving license. |
Form 4 |
Form 4 is also known as the ‘Application for license to drive’. As the name suggests, Form 4 is an application through which the applicant can apply for a license in their state of residence. In the form is a section which states the passing or failing candidates of the driving license exam. |
Form 8 |
Form 8 is also known as ‘Application for the addition of a new class of vehicle to a driving license’. This application is to be filled by those applicants who want to add a new type of vehicle to the driving license they currently hold. |
Form 9 |
Form 8 is for those people who want to renew their driving license. |

This form is also known as ‘Application for registration of a vehicle’. This form is used by the applicant to register his/her vehicle with the RTO. |
Form 26 |
This form is also known as the 'Application for the issue of a duplicate certificate.' It is used by a candidate who wishes to get a new registration certificate in case their registration certificate is stolen or lost. |
Form 27 |
This form is used when an individual is applying to get a new registration mark on the vehicle. |
Form 28 |
This form is used by individuals who wish to acquire a NOC from the vehicle financier. |
Form 29 |
This form is used in situations where the vehicle ownership is being transferred. |
Form 30 |
This form is used as a report for the transfer of ownership of a vehicle. This form is submitted along with Form 29. |
Form 31 |
This form should be filled by the heir if the vehicle owner has died. |
