In case anyone loses the driving license, they will have to apply for a duplicate license in order to be able to drive.

Register a complaint and obtain a copy of the FIR (First Information Report).
Get an affidavit from a Notary on a stamped paper to prove that you have taken an oath before the executive magistrate or notary public or first class judicial magistrate that your license is indeed lost.
Pay the required fee for the notary service and also for the stamped paper.
Use these documents to apply for your duplicate license.
Carry the following documents: 3 passport-sized photos, a copy of the F.I.R, the affidavit, a copy of the original driving license (if available), ID proof, Age proof, Address proof.

Submit it along with the other documents.
