Name of the Form

Declaration as to a place of Ordinary Residence by an Individual having a Declared Office for the purpose of registering himself/ herself in the electoral roll.

Form - 1

Declaration as to a place of Ordinary Residence by an employee of the Armed Forces (along with specified service qualification) for enrolment as service elector in the previous part of the electoral roll.

Form - 2 Declaration

Declaration as to a place of Ordinary Residence by a member of an armed police force of a State, who is serving outside that State (along with specified service qualification) for enrolment as service elector in the previous part of the electoral roll.

Form - 2A Declaration

Declaration as to the place of Ordinary Residence by an individual working under the Government of India in a position outside India (having service qualification) for enrolment as service elector in the previous part of the electoral roll.
