Electoral Officer verifies the application
If the verification is successful, then a duplicate voter ID card will be issued to you
The electoral office will notify you and on successful verification, you will be issued a duplicate voter ID

Download form EPIC 002 from your state's Chief Electoral Officer's website
Fill up the form and attach all the documents such as address proof, identity and the copy of FIR
Submit the form to the local electoral office
Reference number will be issued to you
You can track your application status through this reference number from your state election office website
The electoral office will verify the form
After the successful verification, you will be notified and you can collect your duplicate voter ID from the electoral office
The form that is required to be submitted by the individual for issuing a duplicate voter ID is Form EPIC-002. It is available on the Chief Electoral Officer's website for each state and it is also available in the electoral offices.
Specific details have to be provided while applying for the duplicate voter ID card like the name of the state or union territory, full name, address, date of birth and the reason why you are applying for a duplicate card.
