An application for a passport or any passport related service requires the submission of relevant supporting documents. Primarily, a government-issued form has to be filled stating the type of passport application and other required details.
The Passport Seva portal has the online application form for a passport and can be accessed from the website at www.passportindia.gov.in.This form can be filled in two different ways. One can fill up the e-form and submit it online by logging into the portal. Alternatively, one may download the e-form, fill it up offline and upload it back later at the link provided on the portal. The online process is easiest, quickest and the most convenient one.
Offline passport forms can be accessed in two ways. One can download the form from the portal, print it and fill it manually. It can also be purchased from the Passport Seva Kendra (PSK), Passport Office (PO), District Passport Cell (DPC), Speed Post Center (SPC), Citizen Service Center (CSC). However, very few of these centers process offline applications these days.
Online forms are submitted online through the website whereas printed or purchased forms can be submitted at the PSKs or other collection centers. The fee payment procedure also depends on the mode of submission..

Annexures to be attached to the passport application include affidavits, declarations, specific certificates, and specific letters. These are submitted along with the passport applications wherever required. They can be accessed, downloaded and printed. The various types of annexures found on the passport portal are:
Annexure A - affidavit for submission by illiterate applicants to support their birth date.
Annexure B - identity certificate required to be submitted by all central or state government employees, employees of statutory bodies and public sector organizations. This is also applicable to the spouses and minor children of such employees.
Annexure C - affidavit for submission of passport application for minors by either of the parents who are separated but not yet divorced.
Annexure D - affidavit to be submitted by a lady who is applying to change her name/surname post marriage.
Annexure E - affidavit for changing the name through a deed or an affidavit.
Annexure F - verification certificate for tatkal applications, to be verified for authenticity.
Annexure G - a declaration to be submitted for a minor's passport where only one parent has consented to obtain a passport.
Annexure H - a declaration to be submitted for a minor's passport by both parents.
Annexure I - the standard affidavit for the applicant to state his/her name, parents' and spouse's name (if any), residential address, and then declare that he/she is an Indian citizen who is free of any criminal record.
Annexure J - a specimen verification certificate to be submitted with the standard affidavit (annexure i). The heads of apex business organizations issue it to the directors or owners or partners of its member companies.
Annexure K - a specimen affidavit for a change in passport particulars due to marital reasons.
Annexure L - a specimen affidavit while applying for the replacement of a damaged or lost passport.
Annexure M - no objection certificate (NOC) issued by a ministry/department/office stating that they have no objection to the applicant obtaining the passport.
Annexure N - a prior intimation (PI) letter submitted by employees of a government/public organization/statutory body to inform the employer that they are applying for a passport.
Authority letter - an authorization letter filled by an applicant to authorize a person to submit his/her passport application on his/her behalf. It could also be used to authorize some other person to collect the passport.
A statement of undertaking submitted on plain paper containing the applicant's name, parents' names and spouse's name (if applicable). It also contains the applicant's current residential address, the passport file number or ARN. It also states if the applicant has a criminal record and other relevant details.
