If an individual wants to change his/her middle name or surname, he/she must apply for a re-issue' of Passport by either visiting the Passport Seva Kendra or logging in to the official website of Passport Seva. Moreover, Form No. II for Miscellaneous Services that can be downloaded from the website must also be submitted along with the form. The applicant must submit the following documents:
Old Passport in original along with self-attested photocopies of the first and last two pages of it.
A lady who wishes to change her name after marriage must submit a copy of her spouse's passport and an attested copy of the Marriage registration Certificate (or any supporting evidence such as a joint photo) and an equivalent affidavit as per Annexure D.
A divorcee looking to change her name must submit a certified photocopy of the Decree of Divorce an affidavit or Deed Poll as per Annexure E.
If someone wants to change the name post remarriage, they must submit an attested copy of the Divorce Decree or Death Certificate with/of the previous spouse, along with the proof of second marriage (marriage registration certificate) must be provided.
If the reason for a change in name exceeds the circumstances mentioned above, then an affidavit or Deed Poll as per Annexure E is to be submitted along with original advertisements in two daily newspapers announcing the change of name (accepted only after 30 from the date of publishing the said advertisement).
The documents mentioned above must either be submitted online by uploading the scanned copies to the portal or at the Passport Seva Kendra on the specific appointed day. The rectified Passport will be sent to the applicant's address via post within a few days.
