LPG is one of the most popular and efficient cooking fuel. The convenience, safety and efficiency provided by LPG cannot be compared to any other alternatives it has in today's world. It also has very less impact on the environment which makes it a green cooking fuel.
LPG connection was difficult to obtain due to the lengthy procedure involved along with the high cost of the cylinders. However, the government has come up with many schemes and the gas companies have subsidized the cooking fuel, making it affordable to a large chunk of the population.
If you wish to apply for a Bharat Gas LPG connection or if you want to avail the gas subsidy scheme, there are specific forms that you'll be required to fill out.

This form is to be submitted by individuals who wish to apply for a Bharat Gas LPG connection, containing their contact details and LPG distributor details. |
Ujjwala KYC – Undertaking for availing loan |
Individuals who wish to avail of a loan to cover the cost of the cylinder/gas stove/regulator have to submit this form. |
Declaration of Income for Availing LPG Subsidy |
To avail of the LPG subsidy, individuals have to submit this form, which details their income to determine if they are eligible for the subsidy. |
Simplified KYC form |
This form is to be submitted by individuals who do not possess KYC documents but wish to avail of a Bharat Gas LPG connection. |
Unified PAHAL (DBTL) Joining Form |
This form is to be submitted to register for the direct debit subsidy scheme, through which the subsidy amount would be transferred directly to the individual’s bank account. |
Form 1- Bank Account-Aadhaar Linkage Application Form (for LPG customers) |
This form is to be submitted as part of the subsidy scheme, in order to link the Aadhaar card to the individual’s bank account. |
Form 2- LPG Linking Form |
This form is to be submitted to the Bharat Gas distributor to notify him of the individual’s details as part of the linking process. |
Form 3- Mandate for Non-Aadhaar based LPG Subsidy Transfer |
Individuals who are applying for the subsidy without an Aadhaar card can submit this form to their bank branch in order to receive the subsidy amount into their bank account. |
Form 4- Mandate for Non-Aadhaar based LPG Subsidy Transfer |
Also for individuals who are applying for the subsidy without an Aadhaar Card, this form is to be submitted to the LPG distributor to inform him of the individual’s KYC details to facilitate the subsidy amount transfer. |
Form 5- Mandate for Giving up LPG Subsidy |
Individuals who wish to give up their LPG subsidy are to submit this form to their Bharat Gas distributor. |
Form 6- Grievance Redressal Form for PAHAL (DBTL) Scheme |
Individuals who wish to lodge a complaint or grievance regarding a facet of the direct debit subsidy scheme can do so by filling out this form. |
Unified Form for Transfer/Regularisation of LPG connection |
This form is to be submitted if the individual in whose name the LPG connection was taken wishes to transfer it to next of kin/the connection is to be transferred in the event of the death of the connection holder. |
Declaration for the Loss of Subscription/Termination Voucher |
This form is to be submitted if the individual misplaces the original LPG subscription or termination voucher. |
Declaration for New Connection or Connection against Termination Voucher |
This is a document to be submitted if the individual wishes to register a new connection against a previous LPG termination. |
Format for Reactivation of LPG Connection |
This application is to be submitted to reactivate an LPG connection that was deactivated. |
